PROVIDER AGREEMENT

Providers will be required to download and complete the entire Provider Agreement and Attachments AA – FF for the Rural Nevada Contracts with DCFS.  It will be a requirement for all Specialized Foster Care Home’s to complete and return the Provider Agreement to DCFS before July 15, 2009.  Please check with Clark County and Washoe County for completing Contracts for these Nevada jurisdictions.

SCHOLARSHIP FORMS
Otto A. Huth Scholarship Trust Fund

This fund is available to children in custody of the Nevada Division of Child and Family Services who meet the following criteria: the child must have been in our custody in a foster or group home and must be graduating from high school or receiving the GED the year of application. Deadline for application is April 15th of each year.

CASA Foundation
 provides up to $500.00 per semester to foster youth in Nevada and sometimes to other Interstate Compact Placed Youth also. For more information call CASA at (702) 455-4306 and form can be found online at their Website.
